HBF / MBF: Parental Consent Form

Attention: all HBF/MBF under 18 yrs attending the Purdue International Conference!

Please download attached form and fill out the appropriate information. All HBF/MBF who’s parents are NOT attending the conference MUST have this form filled out and sent to Sam Toh (HBF) bigtoh9@yahoo.com or Yvette Shin (MBF) yvetteshin@yahoo.com.

Without this form, Purdue will not treat underage children in the event of a medical emergency. Please have this completed before next Friday and send in asap.
